The Interprofessional Seniors Outreach Program is a four pillar program. It leverages an online education resource that supports students in enhancing the quality of life of seniors in the Toronto area through patient-centred care.
“It is wonderful to see the next generation of clinicians already working together as a team to improve patient outcomes,” said Maureen Charlebois, Chief Nursing Executive & Group Director, Clinical Adoption, Canada Health Infoway. “By incorporating digital health resources into education, the team is improving the learning experience and paving the way for senior- centred care.”

“We are truly grateful to be the recipients of this prestigious award,” said Jessica Visentin, third-year pharmacy student. “We particularly value the recognition of our initiative to enhance student learning in the context of geriatric care, given the current health care demands of this population.”
“We believe that adopting technology is an effective means of encouraging open discussion and problem-solving through interprofessional collaboration,” continued Katherine Steckham, second-year medical student. “By supporting online discussion and optimizing opportunities for learning, we aim to positively impact students' field-based, in-person encounters with seniors in the community.”
The award is a component of Canada Health Infoway’s Next Generation: Clinicians in Training and is presented in partnership with the Association of Faculties of Medicine of Canada (AFMC), the Association of Faculties of Pharmacy of Canada (AFPC) and the Canadian Association of Schools of Nursing (CASN). The program is designed to prepare a new generation of physicians, pharmacists and nurses for practice in modern technology-enabled clinical environments.
